By signing this waiver, I acknowledge and agree to the following:

1. Assumption of Risk:

I understand that using the fitness equipment and participating in any activities at Shape Fitness involve certain risks, including but not limited to injury, illness, or even death. I voluntarily accept full responsibility for any risks or harm that may occur while using the facilities.

2. Release of Liability:

In consideration for being granted a one-day pass to Shape Fitness, I, on behalf of myself, my heirs, executors, and assigns, hereby release and discharge Shape Fitness, its owners, staff, and agents from any and all liability, claims, demands, or causes of action arising out of my use of the gym and participation in any activities.

3. Gym Rules and Conduct:

I agree to follow all Shape Fitness rules, staff instructions, and posted guidelines. I understand that failure to do so may result in immediate termination of my access without refund.

4. Health Statement:

I certify that I am in good health and have no medical conditions that would prevent me from safely participating in physical activities at Shape Fitness. I understand it is my responsibility to consult a physician prior to starting any new exercise program.

5. Minors:

Anyone under 18 must have a parent or legal guardian sign below.
